Perhaps, have you calibrated your system against some of the tests Phil has done? Before I went about applying the tests to my film/developer of choice I calibrated my exposure/development combination against a test Phil had done. Once I knew I had the same results, I went on my own.
It is possible that your film/developer combination requires more exposure than the manufacturer's rating. I am inclined to think that you did underexposed it, but without knowing if you have calibrated the system an answer to your question is difficult.
