The enlarger is only a source of light. You should expose the film you are testing in contact with the step tablet, if you plan to do contact prints or in the film transport if you plan to do your printing by projection. If you plan to print by projection then place the step tablet on the film platform AFTER you have determined your 4 EV exposure. Make sure the is no flare or light exposing your film from the edges of the step tablet.
As long as all the steps are projected on the film, it does not matter if they are not in focus. Remember the important thing as Phil states in his book is to have only 2 or 3 steps clear, so that you do not loose the toe and the shoulder of the curve.
